CONFIDENTIALITY
All personal information revealed to Access Wellness Center is confidential. The fact that you are in therapy or receiving other services, nor any details of those services will be revealed without your permission. Information concerning our professional relationship can only be released if you sign a “Release of Confidential Information” form. This confidentiality has the following exceptions as provided by law:
1. In the event of a medical emergency, emergency personnel or services may be given necessary information.
2. In the event of a threat of harm to oneself or someone else, if that threat is perceived to be serious, the proper individuals must be contacted. This may include the individual against whom the threat is made.
3. In the event of suspected child or elder abuse, the proper authorities must be contacted. The actions do not have to be witnessed to be reported.
4. If ordered by a judge, other judicial officers, or investigation authorities information regarding your treatment must be disclosed.
5. If records are subpoenaed by an attorney in the State of Washington, they will be released unless you file a Protection Order within 14 days of the subpoena. 6. In the event of death or disability, the information may be released if the patient’s personal representative or the beneficiary of an insurance policy on the patient’s life signs a release authorizing disclosure.
THIRD PARTY COVERAGE
If you plan to use insurance, government or a managed care plan to cover all or part of your treatments, we should discuss some of the features of your plan or insurance. These factors could include caps or limits on reimbursement by your health plan. It is important that you are aware of these potential factors before the decision is made to seek authorization or reimbursement for services through a third party plan. Any co-pays will be collected before service. Many insurance companies reimburse clients for the expenses incurred by using out-of-network providers. If you wish to do this we will provide you with an invoice that will reflect your payments to ACCESS WELLNESS CENTER which you may use to seek such payments. This does not change the basic nature of your financial responsibility for payments on all services provided.
FEES AND PAYMENTS
The fee for an Assessment by a Team of Professionals (ATP) is $150 and includes a 50 minute session with our Team of Professionals followed by a Report with suggested and recommended Treatment options or other Services provided which will be suited especially for you depending on your current condition. Before the session you will be required to fill out a Questionnaire in regards of your Health and Specific Situation. We highly recommend that you give us permission to work with your Primary and Special Care Providers. We also may recommend that any X-rays, blood tests or other information which may be pertinent to your case to be supplied at this session in order to help us better serve you. All services provided by Access Wellness Center must be paid in advance except Third Party Coverage. There is no refunds on any of the services already provided by Access Wellness Center.
RESPONSIBILITIES AND ETHICS
Effective treatments require a collaborative working arrangement between patients and therapists and a clear understanding of rights and responsibilities. For you, it means a commitment to invest time, money, and effort in working toward personal health. In order to meet your treatment goals, it is essential that you communicate honestly and participate actively in therapy. We do our best to provide exceptional professional services tailored to your specific needs. We utilize all of our experience and training to work with you to achieve your stated goals. You have the right to discuss any concerns during the course of any services. If you would like to cancel an appointment please, give us at least 24 hour notice. For all other cancelled appointments there will be $25 charge. Pre-paid appointments cannot be cancelled or rescheduled if less than a 24 note is given and there will be no refunds.
